What is the Medicare Prescription Payment Plan?

Starting in 2025, if you have a Medicare drug plan or a health plan with drug coverage, this option is available. This Plan offers a new way to handle your prescription drug costs year-round. How does it work?

For Part D prescriptions (including mail order and specialty pharmacies), instead of paying at the pharmacy, you’ll get a monthly bill from your insurance. Your monthly payment combines the costs of prescriptions you get and any unpaid balance from the previous month, divided by the remaining months of the year. This formula applies to all plans. Keep in mind that your payment can change each month, so you may not always know the exact amount in advance. If you get new prescriptions, your payment might increase, as there are fewer months to spread the costs.

Over a calendar year (January through December), you won’t pay more than you would out of pocket at the pharmacy or the plan’s $2,000 out-of-pocket limit for 2025. Medicare Prescription Payment Plan: Automatic Renewal Starting in 2026

Beginning with the 2026 plan year, participation in the Medicare Prescription Payment Plan automatically renews from year to year, so long as you remain in the same Part D or Medicare Advantage plan and don’t ask to leave the program. In fact, Medicare’s own fact sheet explains that your plan will renew your participation each year unless you change plans or contact the plan to opt out.

You’ll still receive a notice from your plan reminding you that you’re in the program and explaining the upcoming year’s terms. If you decide the payment plan no longer fits your budget, you can opt out and go back to paying your full out-of-pocket costs at the pharmacy counter. If you switch to a different drug plan for a new year, automatic renewal doesn’t carry over—you’ll need to re-elect the payment plan with your new carrier if you want to keep spreading payments across the year.
