Several Medicare Advantage (MA) plans from Aetna, plus one Aetna-affiliated Part D plan, will not be offered in California for 2027. If you’re enrolled in one of the plans below, your coverage ends December 31, 2026, and losing a Medicare Advantage plan this way gives you a guaranteed-issue right to switch to Medigap without answering health questions. Here’s the full list of affected plans and what to do next.
Which Southern California Medicare Advantage Plans Are Ending for 2027?
The table below lists the specific plans, identified by their CMS Plan Benefit Package (PBP) number, confirmed to leave the Southern California market for 2027. Check your Medicare card or Annual Notice of Change against the PBP number to confirm whether your plan is affected.
| 2026 PBP | 2026 Plan Name | 2026 County Footprint |
|---|---|---|
| H0523-092 | Aetna Medicare Signature Extra (HMO) | San Diego |
| H4982-004 | Aetna Medicare Signature (HMO) | San Diego |
| H5521-422 | Aetna Medicare Signature (PPO) | San Diego |
| H5521-580 | Aetna Medicare Enhanced (PPO) | San Diego |
| H5521-419 | Aetna Medicare Signature (PPO) | Los Angeles, Orange |
| H5521-578 | Aetna Medicare Enhanced (PPO) | Los Angeles, Orange |
| H5521-478 | Aetna Medicare Enhanced (PPO) | Santa Barbara |
| H5521-292 | Aetna Medicare Enhanced (PPO) | Ventura |
| H5521-369 | Aetna Medicare Eagle (PPO) | Alameda, Los Angeles, Orange, Sacramento, San Diego, San Francisco, San Luis Obispo, San Mateo, Santa Barbara, Santa Clara, Ventura, Yolo |
| S5601-064 | SilverScript Choice (PDP) | Statewide in California |
The SilverScript Choice (PDP) listed above is a standalone Part D drug plan, not a Medicare Advantage plan. If that’s the only affected coverage you have, you’ll need to pick a new Part D plan during Medicare’s Annual Enrollment Period, but losing a stand-alone drug plan does not, by itself, open a guaranteed-issue window for Medigap.
You Have a Guaranteed-Issue Right to Switch to Medigap
If your Medicare Advantage plan is one of the nine listed above, federal law gives you a guaranteed-issue right to buy a Medigap policy with no medical underwriting, as long as you apply within 63 days after your coverage ends. You can see Medigap pricing here.
Since these plans end December 31, 2026, that window runs through early March 2027, but we recommend applying by December 31 so your new Medigap coverage starts January 1 without a gap. For the full breakdown of which Medigap plan letters you qualify for and mistakes to avoid, see our Medicare Advantage plan termination guide. Note: You also have the right to switch to another Medicare Advantage plan. The best time to do that is between Oct 15-Dec 7.
💡 Tip: Keep your plan’s non-renewal notice or Annual Notice of Change. It’s your proof of guaranteed-issue eligibility when you apply for Medigap. Source: Medicare.gov.

What Happens If You Don’t Switch?
If you don’t choose a new plan, you’ll automatically move to Original Medicare on January 1. Original Medicare covers about 80% of most Part B costs with no yearly cap on the remaining 20%, and you’ll lose your Medicare Advantage plan’s built-in drug coverage, so you’ll also want to enroll in a standalone Part D plan during AEP to avoid a late enrollment penalty.
Next Steps If Your Plan Is Ending
- Confirm your plan’s PBP number matches the table above, or check your Annual Notice of Change (this should be sent to you in the mail)
- Save your non-renewal notice as your guaranteed-issue proof (sometime called “right to buy” letter.
- Compare Medigap plans and apply before December 31, 2026, for a clean January 1 start
- If SilverScript Choice (PDP) is your only affected plan, pick a new Part D plan during AEP instead; it doesn’t trigger Medigap guaranteed issue
- Call our team if you’re not sure which situation applies to you
